The Regulator has released an animation and investigation report detailing a dangerous incident at Attunga Limestone Mine in which a worker was trapped between the cab and chassis of a wheel loader.
On 30 November 2020 a contracted technician was replacing failed cab mounts on a Volvo L220H. He was tightening harnesses and hose straps under the tilted cab when it suddenly lowered onto his upper body.
The worker suffered minor crush injuries however the incident had a high potential for a serious or fatal injury.
The Regulator has identified several ineffective risk controls and causal factors, and makes a range of recommendations to manufacturers, inspectors and repairers of mobile plant, and workers.
